Bleeding gums are the body’s first warning sign of inflammation in the gum tissue. Long considered a minor issue caused by poor brushing technique or a generic buildup of tartar, medical science now understands this symptom as a reflection of dysbiosis, a deep imbalance in the oral microbiome. When gingivitis progresses to chronic periodontitis, the resulting loss of bone and dental support can put the patient’s overall health at risk. In these cases, traditional cleanings and generic mouthwashes are often not enough. Modern dentistry has taken a major step toward precision medicine through the use of advanced microbiological testing.
What causes bleeding gums
The mouth is home to billions of microorganisms living in balance. But poor hygiene, stress, or genetic factors can allow highly destructive anaerobic bacteria to take over. Microorganisms from the so-called “red complex” — Porphyromonas gingivalis, Treponema denticola, and Tannerella forsythia — hide in the crevicular fluid, the space between the gum and the tooth. These bacteria break down the epithelium, damage capillaries, and cause bleeding. Chronic gum inflammation doesn’t stay confined to the mouth, either: it’s closely linked to systemic conditions such as diabetes and cardiovascular disease through what’s known as the “mouth-gut axis”.
The latest molecular diagnostic techniques in periodontics
At Clínica Dental Smiling, we no longer treat infection blindly. To map out each patient’s bacterial profile, we use two molecular techniques.
- Quantitative PCR (qPCR): Small, sterile paper points are painlessly inserted into the periodontal pockets to collect a fluid sample. Within just a few days, dental labs amplify the bacterial DNA to identify exactly which pathogens are present and in what concentrations.
- 16S rRNA gene sequencing and metagenomics: The most cutting-edge technology available. It allows the entire oral microbiome to be sequenced in real time. Unlike traditional lab cultures, where many anaerobic bacteria used to die on contact with oxygen, genetic sequencing can detect even the hardest-to-culture species at scale.
Personalized treatment for bleeding gums
Modern treatments for bleeding gums target the real problem, delivering faster, more reliable results:
| Traditional approach | Modern microbiological approach |
| Blind mechanical cleaning (general scaling). | Decontamination guided by the bacterial map. |
| Generic broad-spectrum antibiotics. | Targeted antimicrobial therapy (only the specific antibiotic needed). |
| Visual monitoring of inflammation and bleeding. | Molecular monitoring to confirm the pathogen has been eradicated. |
With an exact microbiological profile, we can stop the bleeding from the very first sessions and avoid the indiscriminate use of antibiotics, helping to reduce bacterial resistance. We also help prevent peri-implantitis — the loss of support around dental implants — before any visible symptoms appear.
If you notice bleeding when brushing or flossing, at Clínica Dental Smiling we identify exactly which bacteria are causing it before deciding on any treatment.
